Staff Health Assessment

Employee Cost: Determined by insurance company and doctor
To Be Completed By: Before Paperwork Meeting   

• Doctor must fill out attached form completely and stamp the bottom with their office stamp
• TB Test must be done and dated within 1 year of hire

Work Verification Form

Employee Cost: $0
To Be Completed By: Before Paperwork Meeting

• Prior employer from any child care related job should complete the form
• Employee should have at least 2 years of experience 

State Police Criminal Background Clearance

Employee Cost: $22
To Be Completed By: Before Paperwork Meeting

• Go to https://epatch.state.pa.us/Home.jsp
• Click on “Submit a New Record Check” and choose “Individual Request”
• Complete all required fields and click next
• Click on “Proceed”
• Complete required fields and click enter this request
• Click on “Finished”, “Submit” and “Checkout”
• Fill in payment information
• Certificate should be available to print immediately in most cases

Child Abuse Clearance

Employee Cost: $13
To Be Completed By: Before Paperwork Meeting

• Go to https://www.compass.state.pa.us/cwis/public/home
• Click on “Create Individual Account”
• Complete all fields; create your own Keystone ID
• Then Log in using the account that you created
• Complete all required fields
• Certificate will be emailed to you within 2-7 days.

FBI Fingerprint Clearance

Employee Cost: $23.85
To Be Completed By: Must show receipt at paperwork meeting

• Go to https://uenroll.identogo.com/
• Enter the service code “1KG738”
• Click on “Schedule Appointment”
• Complete required fields
• Create an appointment to go get your fingerprints
• Print your receipt
• Certificate will be mailed to you within 1-3 weeks

Mandated Reporter Training

Employee Cost: $0
To Be Completed: Within 7 days of hire
Employee Paid: 3 Hours of training  

• Go to http://www.reportabusepa.pitt.edu
• At the top, click on “registration”
• Complete all required fields
• Log in with your user name and password
• Complete training and print certificate

Health and Safety Training

Employee Cost: $0
To Be Completed: Within 30 days of hire
Employee Paid: 10 Hours of training  

• Go to https://od.bkc.psu.edu/user/sign_in
• Log in or create an account
• Click on “required health and safety” on the left hand side
• Click on the course titled “Get Started with Center-Based Care: Building Blocks for Quality”
• Complete the training and print the certificate

Stars 101 Training

Employee Cost: $0
To Be Completed: Within 30 days of hire after paperwork meeting
Employee Paid: 3 Hours of training  

• Go to https://papdregistry.org/default.aspx
• On to top of the page click on “Stars 101 and 102”
• Scroll down to “STARS 101 Online Self-Paced” and click on view events
• Click on “register online”
• Login using your account
• Complete the online training

Employee Paid: 3 Hours of training

• Complete the online training
